The Initial Concerns Assessment can be completed by the class teacher to begin identifying the child/ young person’s areas of strength and difficulties.
Any identified quick wins, strategies or interventions should form part of the assess, plan, do, review process.
If concerns persist, further exploration is required into the young person’s Learning Environment and Developmental Environment.
These should be completed in collaboration with the class teacher and SENCo to identify what is helping and hindering the young person within the school and home environment.
It is important to gain the young person’s views and the views of their parents/ carers to develop a holistic overview of the young person’s strengths and needs.
My Views About School can be used to explore the young person’s views on the school’s physical, social and learning environment. Please read the guidance for using this resource.
The Family Views Questionnaire can be used to support a shared understanding between home and school. Guidance for using this resource can be found here.
If concerns continue to persist, further exploration may be required. These two assessments are optional.
If the young person appears to be experiencing difficulties with their working memory, flexible thinking, maintaining their attention and impulse control, the Executive Functions Assessment can be used to further explore these areas.
The Behaviour Frequency Count can be used to record and identify patterns of behaviour.
