INclude Step 3 Referrals

Referrals should only be made after schools have:

• Followed Steps 1 and 2 of the Nottingham City Council inclusion process.
• Applied Ordinarily Available Provision.
• Completed two Assess–Plan–Do–Review (APDR) cycles for the child.
• Sought training and advice from other services where appropriate.
• Adapted provision based on identified needs.

Schools may consider referral if:

• There is an increase in behaviour logs or severity of incidents.
• The child has received detentions or suspensions.
• The child is at risk of permanent exclusion without additional support.

At the point of referral, schools may feel they need:

• Specialist advice to reduce the risk of suspension and permanent exclusion.
• Professionals from the INclude Service to work alongside them to support the inclusion of the child.

Referrals must be made by schools only.

1. Complete a DASH referral

2. Attach the following compulsory documents:
• Download Pupil Support Summary Template
• Download TAF Form Template
• Download HLN Bid Template - primary
• Download HLN Bid Template - secondary
3. If applicable, also include:
• Download Risk Assessment Template
• Download Individual Handling Plan Template

• Schools must seek parental/carer consent before applying.
• INclude Service Privacy Notice
